Job Description

Patient Coordination and Navigation

  • Serve as the primary liaison between patients, families, physicians, nurses, and other healthcare professionals.
  • Coordinate the patient journey from registration, consultation, investigations, treatment, admission, discharge, and follow-up care.
  • Facilitate timely scheduling of consultations, procedures, surgeries, diagnostic tests, and specialist referrals.
  • Coordinate patient transfers between departments and clinical units as required.
  • Ensure patients and families are adequately informed regarding treatment plans, procedures, timelines, and care pathways.
  • Assist patients in navigating hospital services efficiently.

Communication and Stakeholder Management

  • Maintain effective communication among consultants, nursing teams, diagnostic departments, pharmacy, laboratory, radiology, and administrative services.
  • Liaise with internal stakeholders to ensure smooth execution of patient care plans.
  • Coordinate multidisciplinary discussions and facilitate timely communication of patient-related information.
  • Provide regular updates to patients and their families regarding treatment progress and planned interventions.
  • Escalate concerns, delays, or operational issues to the relevant department heads.

Admission, Discharge and Follow-Up Coordination

  • Coordinate patient admissions, room allocations, transfers, and discharge processes.
  • Ensure discharge documentation, prescriptions, medical reports, and follow-up appointments are prepared in a timely manner.
  • Assist patients and families in understanding discharge instructions and post-treatment care requirements.
  • Arrange follow-up appointments and monitor patient compliance with recommended care plans.
  • Support continuity of care initiatives to improve patient outcomes.

Patient Experience and Service Excellence

  • Promote a patient-centered culture and ensure a positive patient experience throughout the care journey.
  • Address patient concerns, inquiries, and service-related issues promptly and professionally.
  • Support patient satisfaction initiatives and contribute to service improvement programs.
  • Assist in handling patient feedback, compliments, and complaints in collaboration with the Patient Relations team.
  • Maintain confidentiality and uphold patient rights at all times.

Insurance and Administrative Coordination

  • Coordinate with insurance companies, third-party administrators, and corporate clients where applicable.
  • Facilitate pre-authorizations, approvals, and documentation required for insured patients.
  • Ensure completeness and accuracy of patient information and records.
  • Maintain appropriate documentation and reports related to patient coordination activities.

Clinical Support

  • Collaborate with healthcare teams to ensure continuity and quality of patient care.
  • Monitor patient progress and facilitate access to required services.
  • Assist in organizing health promotion activities, patient education sessions, and wellness programs.
  • Support vaccination campaigns and preventive healthcare initiatives when required.

Quality and Compliance

  • Ensure compliance with hospital policies, patient safety standards, and regulatory requirements.
  • Participate in quality improvement initiatives aimed at enhancing patient satisfaction and operational efficiency.
  • Contribute to the implementation of best practices in patient care coordination.
  • Participate in departmental meetings, training sessions, and continuous professional development activities.
