We are seeking a dedicated and compassionate individual to join our team as Outreach Social Worker. This role involves providing essential support, education, and advocacy for individuals with haemophilia and other bleeding disorders, as well as their families.

 

Key responsibilities and tasks include:

  • Conduct home and hospital visits to individuals with haemophilia and other inherited bleeding disorders to enhance self-confidence and self-esteem.
  • Provide comprehensive support in various aspects of life for individuals with haemophilia and other bleeding disorders through education, advocacy, and assistance.
  • Offer information about bleeding disorders to newly diagnosed patients and their families at different life stages.
  • Provide support to immediate family members of patients.
  • Refer patients to other NGOs and service providers as needed.
  • Conduct family pedigree assessments and liaise with family members to understand the needs of each patient.
  • Serve as an emotional support system by offering counselling to patients and referring them to psychologists when necessary.
  • Act as a liaison between patients, families, and healthcare professionals.
  • Prepare weekly reports on scheduled visits, including recommendations.
  • Maintain positive relationships with stakeholders, promptly reporting any issues to the manager.
  • Ensure that each patient receives a haemophilia bracelet and make arrangements for its provision.
  • Collect data at each regional hospital.
  • Monitor the budget.
  • Perform any related duties as required.

Person specification Skills and experience Essential:

  • Diploma or degree in social work or a related field.
  • Ability to communicate effectively with stakeholders at all levels.
  • Availability to work on Sundays and public holidays.
  • Willingness to work after office hours and at night.
  • Effective time management skills and ability to meet deadlines.
  • Ability to work independently and take initiative.
  • A driving licence with own vehicle would be an advantage.
